Sales tax will be charged at the rate of 6.25% for all Ohio purchases unless a tax exempt number is
submitted.

All customers using a purchase order number must have a signed tax exemption or resale certificate
on file at our office. Area provided on shopping cart check out for purchase order information. A copy
of the purchase order must be faxed to D&G Uniforms at 330-798-9960 before order is processed.

Most items are in stock and will be shipped U.P.S. ground or US Postal Service.
Allow 4-6 weeks delivery for customized items and badges.

Badge prices include shipping charges.
Orders over $1000.00 call for shipping charges.
